Better nylon rollers run more balls in a sealed race, so nothing gets in and nothing needs to get out.
At the bottom of the range are solid plastic rollers with no bearing at all. They are quiet for a season and then develop flat spots where the door sits, after which the door thumps at the same point on every cycle. If you can spin a roller by hand with the door closed and it either grinds or refuses to move, that roller is finished regardless of what the wheel looks like.
The Stem Is Usually the Real Fault
Behind the wheel is a steel stem that sits in a sleeve on the hinge, and that is where most seizing happens. Corrosion builds up between stem and sleeve, the roller locks solid, and from then on the wheel skids along the rail rather than turning. Nothing about the wheel will look wrong. The wear shows up on the track instead, as a polished stripe.
Stem length matters too. Standard hinges take one length, and the top fixture takes a longer one so the door meets the header properly. That top fixture is adjustable and almost never adjusted, which is why so many older doors seal along the bottom but stand proud at the top. Putting the wrong stem in a hinge leaves the roller sitting too shallow in the rail, and a shallow roller is how a door comes off.
Alley Grit, and Why the Track Gets Wiped Rather Than Greased
Cars come in wet all winter, carrying salt and whatever the alley surface has thrown up. The film settles on the rails and on every open bearing in the garage. The instinctive response is to spray grease into the track, and that makes it worse: grease holds the grit against the roller and the mixture works like a lapping paste.
The track wants a rag and a clean cloth, not a can. Lubricant belongs on the stems, in the hinge pivots and on exposed bearings, where it displaces water and keeps corrosion off the steel. Rollers get changed as a full set with the door closed and the springs at rest, working hinge by hinge, and the door is then run by hand before the opener goes back on it.

How many rollers are on a door, and do they all get replaced?

Usually, yes. Rollers are among the least expensive parts on the door and they change how the whole system behaves, so an otherwise sound older door gains quiet operation and takes less abuse through the hinges and track. Where it is not worth it is a door with rotted or badly bent sections, or one whose hinges have worn their holes into slots. In that case the technician will say so rather than fit new parts onto something at the end of its life.
