Two Tests That Tell You Whether a Door Is Safe

First, lay a length of two-by-four flat on the floor under the middle of the door and close it. When the bottom rail meets the wood the door has to reverse and travel back up. Second, close the door and pass a broom handle through the beam between the two sensors near the floor. It has to stop and reverse immediately. A door that fails either test should not be left in service.
Openers older than the photo-eye requirement
External photo-eyes became a requirement on residential openers in the United States in 1993. A fair number of doors on Merrillville's original streets are still driven by units that predate that, or by units where somebody tired of the beam interrupting the door and screwed the sensors up under the ceiling pointing at each other, which defeats the entire purpose.
An opener with no obstruction detection is relying solely on a force-reversal setting that has drifted over thirty years. Those are the ones we most want to see, and they are also the ones where the honest recommendation is a new operator rather than an adjustment.
What cannot be seen from the floor
Standing in the garage you can see roughly half of what matters. The rest needs a ladder and a light: the lift cables where they wind onto the drums, corrosion pitting along the spring coils, play in the end bearing plates, the lag bolts holding the horizontal track hangers into framing, and whether the header bracket carrying the opener arm has begun to pull away.
On an extension-spring door, the first thing checked is whether a containment cable runs through each spring. On a torsion door it is the center bracket and its lag bolts, because that single fitting holds the whole counterbalance against the header.
What you are left with at the end of it
A clear picture of the door split three ways: what is unsafe and should not be used until it is corrected, what is worn and will want attention within a season or two, and what is fine and can be left alone.
Nothing gets repaired during an inspection without being explained and approved first, and if a door passes we say so and leave. Pre-sale calls are common here. Home inspectors test the reversal as a matter of routine, so it is an easier conversation to have before the report is written than after.

How long does an inspection take?

My door reverses on the two-by-four, but slowly. Is that a pass?
Not really. The reversal has to be prompt. A lazy reverse normally means down force has been turned up to overcome a door that is out of balance, which is a counterbalance problem being masked by an opener setting. Correcting the balance is the actual repair.
Do rental and managed properties need checking more often?
It is a sensible habit for any door used by people who did not install it and have no reference for how it used to sound. Tenanted homes and shared parking structures see harder use and far less reporting, so a yearly check catches things nobody has thought to mention.
